Due to Covid-19 we are conducting registration using the following procedure.
1. Parents complete registration in Final Forms located on our District Website under the parent tab (if not able at this time please complete the attached form to hand in with the items below).
2. Parent(s) must schedule an appointment with the office during the month of May. Please bring the following items to your appointment:
- Student birth certificate
- Student social security card
- Proof of Residence
- Custody documentation (if applicable)
3. All student health screening will take place in the fall. Students do not need to accompany their parents to the scheduled visit.
